I see no problem with stamping and signing the first sheet in a place that does not obscure critical information. Plus in the comments section note that the first page is only valid if signed and stamped identically to that on the following page.

My local flood plain manager shared some email conversations she had with FEMA, and FEMA said they will not move the cert to the first page this time around. I think we are stuck with this one until 2018 whenever they feel like fixing it.

Put this to use today and it worked perfectly. I'm not sure why I thought it was not autofilling the location information at the top of each page earlier. Everything worked great.........even insertion of the photos. Thank you very much.

This was brought to their attention at least 4 months ago - they apparently don't care.

I did already a couple of EC utilizing your forms and it works great.
Last time I did it the customer asked me about pages number.
It maybe make sense to renumber pages without instructions so it is not going to be confusing because some customers thinks that something is missing.
Also the option to insert the pictures works great however it doesn't allow to customize it (make it smaller, move to center, or remove it at all) unless I am missing something.

You're correct about it lacking the ability to customize the photo. I found it impossible to make it customizable and still allow users with Adobe Reader to fill out the form. I have a new version off the software that I used to create the form so I'll revisit the issue when I get a chance.

Does the FEMA version allow insertion of photos by users of Adobe Reader?
The new FEMA form contains text in the area for the seal & signature that obscures the Registrant's name... Dumb as hell!
