I decided to start a new thread since the Original was about the form not being fillable.
The attached file is fillable in Acrobat or Adobe Reader and will prompt you for images if you click in the seal area or any of the six large open areas on the photo pages. I left a line under each photo area where you can include a description of the photo above. I also removed the cover pages and instructions to shrink the file from 4mb to about 200kb.
If you find any issues please let me know and I will fix if possible.

Thank you so much for this! One issue/request though.... The directions for Section C2 require "N/A" if a particular field does not apply. Right now you have them set up only as a numeric field, so it's preventing me from typing letters. Am I missing something? Thanks

From what I can tell, this is the cat's meow! It might be my fault, but the only glitch I noticed was that the site address has to be entered at the top of each page rather than automatically grabbing the first entry and repeating it where needed later. But, I have low level Adobe, so that could be the issue.

It does the same thing when I tried to enter N/A also, but other than that its great so far. Inserting an image is so much easier now. Thanks Stephen!

Thanks for the feedback. I'd forgotten about the need to fill unused elevation blanks with N/A so I've changed those from numeric to text fields throughout the form. I also fixed a few tab order mistakes so that the cursor will jump to the fields in order. Mr. Cow, I've double checked, and the site address, city, state, zip should be auto filled once you enter it on the first page. I've set this up to be fully functional with Adobe Reader including the ability to save the filled in form so it might be worth the time to install the latest version of Adobe Reader if your Acrobat won't cooperate. I think I saw that it should work with Acrobat version 7 and up.

Point taken, but I bet once it is printed they won't know the difference. I used their non-fillable pdf and overlaid the fields and buttons so the printed from should be indistinguishable. F'em.

Nothing you can do about it but it was pointed out at the recent PLSO conference that having the surveyor's seal on a separate page from the elevation data was a big mistake on FEMA's part. Perhaps they will come out with a form that fixes this problem.
