Be careful.?ÿ Not every form that pops up online is THE form to use today.?ÿ dms330 has what you need.?ÿ However, always doublecheck that you are getting the absolutely correct form.?ÿ FEMA will bleed you like a stuck hog if you use one that appears to be the correct one but they say is not.?ÿ After sending in the wrong form TWICE they may send you the correct form which has the exact same form number and the exact same form expiration date, like September 2014, but with some rearrangement of text and adding a couple of extra slots to fill out.?ÿ I think they do that just to piss off surveyors.

Their form can't be saved in a editable file. Why would they do that?

Both the file that dms330 posted and the one I downloaded from FEMA can be filled in and saved for future editing in Adobe Acrobat.?ÿ When I want to send one out, I print it as a pdf and password lock the file so that it can be viewed and printed but not modified without taking the time to find a pdf software that ignores Acrobat's password security.

@stephen-wardHis form is pretty primitive. It doesn't auto fill each page with the address. The tabs don't have enough spaces for all the Lat. and Long. data. Inserting photos is very cumbersome.

FEMA is hard enough to deal with. A?ÿ poorly done form sure doesn't help.?ÿ

Is there anyway to make the old form so you can save an editable copy?

I have found it to be just fine if I use Adobe Acrobat Reader.?ÿ I may have had some problems in other programs.

@skeeter1996 My Adobe Livecycle software that I used to tweak the forms previously is out of date and no longer installed so I really don't have the ability to tweak their forms anymore.

As dms330 said above, the current form from FEMA works as good as anything I ever created as long as you're using Acrobat to work with it.?ÿ I save it to my hard drive and open it with Acrobat rather than letting it open it in a browser window.?ÿ I'm able to save filled copies for further editing.?ÿ

@dms330I've tried it with PDF Pro 10. It doesn't autocopy the address to each page, The Font is too small, The space for the Lat, and Long are too small for all the figures. The Photos don't self size to the Photo frame.

The old FEMA form did all of that, you just couldn't save an editable copy.

@skeeter1996 Typically I just click inside the photo field and it will open a file manager type dialogue box that will let you select a photo.?ÿ It complains if the image file is over a certain size.?ÿ I open the photos in Paint and use the rescale tool until I get them down to size.

I stand corrected. I was not using Adobe. I purchased PDF Pro 10. It is an inferior product and couldn't perform the required actions.

The form from FEMA will only properly work with Adobe.
