I've modified the fillable Elevation Certificate to allow Adobe Reader users to save the form with the filled in data. I was also able to insert two image fields and text boxes on each of the photo pages to allow inserting of images and descriptions.
On the photo pages the description fields will be highlighted like all other fields in the document. The image fields are not so obvious, just click above the description field and a dialog will prompt you to select the image to be inserted into the image box. I sized the image box so that a 4:3 ratio image should be more or less centered.

On a side note, it seems every time the gov. makes a new form, there is always some part of the form that doesn't work properly (last time it was length of field when entering coords) now on the new form for building type you can't enter 1A or 1B, only a number.

The field for A7 allows me to input up to 3 characters which can be numbers, letters, or combo. Is there another field for building type that I'm forgetting?
I can change the length and restrictions on most of the fields on the form. If any of you run across issues email me (in profile) and I will try to make additional changes.

I assume you are talking about your program and not adobe? Under adobe 10(x) I can only enter up to two digits on line A7, no letters.

I think FEMA has released (at least) two revisions to the form.
The second release(most current) will allow you to put a letter after the building type number.

Is there a way into insert my stamp into the pdf?
If you have your stamp as a pdf I believe Adobe Reader will let you insert it as a custom stamp. I'll give it a shot with my stamp later today and let everyone know what the procedure is.
Edit: Or I might be able to add image boxes where the stamps go but probably wouldn't be able to control the size of the stamp without fiddling with the size of the original image of your stamp (add whitespace around the perimeter of you stamp to shrink its size in the image box. I'll let you know how it goes.
