Carlson 2015, IntelliCAD 8, 32-bit install on a 64-bit Windows 7 machine.
I have a drawing giving me difficulty with the perpendicular object snaps. I've tried running osnap, pick from middle button, and typed at the command line, all behaving the same. When I try to draw a line or polyline perpendicular to another line (2D, at elevation 0.0000) the indicator shows the correct perpendicular location where the line should snap to, but when I finish the line, it snaps to the point where the pickbox was located on the target line. I've run audit and recover on the drawing, with 0 errors showing.
I then created a new drawing, and inserted the problem drawing. Tried drawing a line perpendicular to another, and it worked fine, so I suspect a setting in the problem drawing is causing this odd behavior. I'd like to fix it, if possible, since it's a large drawing, and inserting it to another one to continue my work might cause other problems to pop up.
I have found that when Carlson has a hiccup it is time to reboot the machine and let the settings align themselves.
Can't really say what causes it, my take is that during use a computer and/or program goes thru some setting changes and options and commands begin to get tripped up on execution.

Solved it. Guy in my office knew what the issue was. Somehow the drawing grid SNAP was set to ON, and we don't know why, but it messes with the perpendicular object snap in IntelliCAD. Turned SNAP off, and it's working fine now.
Some times you'll have an issue snapping to a 3d polyline.. Try drawing a new line over the line your are trying to draw perpendicular to.
I find using "FLATTEN" to be a better solution unless you need the 3D entity.

Thanks, but these were 2d polylines and basic lines with elevation 0 at all vertices. The SNAP setting being ON was the issue. I've been merrily drafting away the past hour since turning it off.
The next time it happens try this.
Instead of drawing from a point perpendicular to the line, draw from perpendicular to the point. In other words draw the line backwards. Sometimes it doesn't work but it's worth a shot.
