I've taken FEMA's fillable pdf and removed the cover page, instructions, etc. I also made it so that you can save the filled form even if you're using Adobe Reader. FEMA actually did a great job on the form. Not sure if they rose to the challenge of my earlier effort or if they finally got someone who knew the software well enough to create fields for the seal and photos that allow you to click and load an image. Those who prefer to wet stamp the form can upload the attached blank seal to the seal field and it will cover the pre-printed text so that field will appear blank when the form is printed. I'll take a look in the next couple of days after I get past my current deadline. Sounds like FEMA restricted them to numbers without thinking of the need to put N/A in the fields if not used. Should be an easy fix.

Thanks for the reminder. I've modified the form so that the A8 and A9 sub-fields allow text to be entered rather than being restricted to numbers only. The updated file is attached below.
I know some would think this is a bad idea, but could you put a "picture" field for the signatures line (like the seal).
Most my customers want a signed PDF, now I print it and rescan it to insert the signed page into the elevation cert.
I'm the only PLS here that even would touch one of these and I keep it under "lock and key".

I probably could insert a photo field for the signatures, but I suspect that it wouldn't work well because of how it scales the inserted images. If you have a smart phone with adobe reader (free) on it you can get one of those rubber tipped styluses and sign right on screen. Or, if you're using Adobe Acrobat on the desktop you can set up signatures with a scanned image of your signature that will do the same thing without all the hassle. Has anyone encountered the leading zero issue with these forms. If you place a leading zero in the decimal portion of the elevation the form drops it (100.01 becomes 100.1). Is there any way to fix this issue.

I hadn't noticed it because I've been rounding everything to the nearest tenth, but yes it does the same thing to me. I'll try to look at it this evening and see why it's doing that. Most likely they tried to idiot proof it by putting rules on the type of data or format that could be entered and it backfired.
It looks like you just need to add a format to the field (99). Wish I had Live Cycle to try it myself.

Unfortunately, I started with their fillable form so I've inherited the baggage that comes with a form designed by someone who's never filled one with real world data. They've used a decimal field which would be great if the form didn't have separate fields for the whole numbers and decimals. I took a quick look, but I can't figure out how to create a pattern that allows but does not require leading zeros. I'll probably have to switch to a straight numeric field or maybe even a text field, but I want to be careful not to create additional issues.

OK I didn't realize that I had access to Live Cycle as part of my Acrobat X Pro install, all I had to do was modify my installation through control panel and add the LiveCycle option....On the form I just had to right click the field for the decimal portion of each elevation and select Pattern->Display and set the pattern to either num{9} or num{99} depending on how many decimal places was needed.

Yeah, that works if you can make changes on the fly, but I'm trying to find a fix for everyone with Adobe Reader. The problem with the num{99} pattern is that if someone enters "1" in the field, it will be displayed as "01". That will get someone in trouble if they're not paying close attention. I'll work on it more later and see if I can find a more user friendly solution.

I do have one comment if you don't mind.
I didn't find the way to split a field you created for photos insertion or add additional page if more than 4 photos are required.
Is that possible to add one more page (the same as page 6) so we could utilize in case if we need to have more photos?
This last page could be disregarded if it is not required.

Very often we have to add more pictures to avoid additional questions. For example if the house is surrounded by a lot of vegetation and we would like to show certain part of the foundation so insurance company could see flood vents, etc.
So I am not sure what would be the best way to add more pictures. It shouldn't be a big deal to leave one page blank if we don't need them
