I got contacted from a firm in another state, basically they want me do do the fieldwork and complete sections 4-9 of Part A and Parts C&D of the form,sign and stamp. All other info will be provided by the "project owner". How much liability am I taking on if another party provides "all other information"?
We got that call too. I told them I certify my work, which means we determine or verify every bit of information on forms we seal...

Yes, you accept 100% liability unless you have a special clause in your E&O policy covering acts of gullibility, ignorance and stupidity.

Seems really bizarre that they want you to fill out elevation sections, sign and seal, but leave things like the address, FIRM map info, BFE, and photos blank, so they can complete them later.
They can't seriously think they are saving you a bunch of time by leaving the address off, and therefore should get a discount, can they?
That proposal smells bad.
My understanding was that they send me the form with the site information already completed. Then I complete and sign the "Surveyor" portion. I figure that will allow me to check everything before signing.
If that's not the case; I won't be involved.
