I have been working on the survey department manual the past several weeks and have been putting together standard forms. Thanks again to those who have shared other forms in the past (survey request form). Today I am working on a field crew daily report form. I have the PC make copies of their field book pages for the day. It has a title page with job #, crew members, equipment used as well as weather conditions and so on. As a redundancy, they all have a journal they fill out and keep with them. At the end of the year or the end of their employment, I get their journals.
I worked at a company where we had the 365 page hard bound daily journals. One guy I worked with was a really funny dude and turned me on to the idea that field notes should "tell the story of the day" as well as document measurements.
His dailies were pretty good especially when things went wrong. I recall one that recounted going to site and getting a ATV stuck, going to the bosses house to get another to rescue and then running out of gas with the rescue ATV.
I like dailies for construction work with carbon copies, white for contractor, pink for office and canary for field, IIRC. Just simple company logo at the top, Crew Name, Job Name, Hours Billed, Tasks Performed and my personal favorite "Task Difficulties".
