Couple of things I noticed...

item a5.
The latitude and longitude coordinates must be accurate within 66 feet. When the latitude and longitude are provided by a surveyor, check the “Yes” box in Section D and indicate the method used to determine the latitude and longitude
in the Comments area of Section D.

items a8.b–d
Enter in Item A8.b the number of permanent flood openings in the crawlspace or enclosure(s) that are no higher than 1.0 foot above the higher of the exterior or interior grade or floor immediately below the opening. (A permanent flood
opening is a flood vent or other opening that allows the free passage of water automatically in both directions without human intervention.) If the interior grade elevation is used, note this in the Comments area of Section D.

I did a straight-up conversion, I did not attempt any reformatting that may be required.
Not sure how to upload to this forum - seems to only allow uploading images.

Note that the original is a fillable PDF, meaning you don't need to convert it in order to fill in the certificate data.

The pdf file is form fillable..do not need in word document

I like that it is fillable.

I just can't figure out how to insert a digital picture into page 3.
In Word, I would copy, paste, resize.

I can insert an image on page 3, but cannot figure out how to resize it. I like the PDF format.

On the last form, my Open Office form would "un-check" the boxes for the chjeck marks for some reason. Very Frustrating.

Yes, but it cannot be saved to hard drive. If I do a construction elevation, with two or three trips, I am going to have to complete a new certificate for each trip. If I have something that I can save to my hard drive I will only have to change a few things.
Also, if I wish to look at something at a later date I will have to pull the folder on it and not just open the file.

Dan,

I was able to edit the PDF, and save it to my hard drive. I did already have a blank copy, and edited it with just generic information, and did a save as. I then opened the new file.

I was able to figure out how to resize the image. I really like this one much better. I need to play around with it a little more, when I have more time.

Is anyone willing to shoot me a word document of the new certificate if they have it converted already? .pdf makes me fill one out everytime and I can't insert pictures to page 3. I do alot of these since I am on the coast and it saves time and effort to have most of it filled out already. Thanks.

I guess I need Acrobat -- not the free reader in order to insert pics?

I downloaded a trial copy of Acrobat Pro and I was able to insert and resize pics.
I just don't want to have to buy it:-(

I just sent one out on Tuesday AFTER visiting the FEMA site to make sure I was using the most current one!!! I bet it comes back...

FEMA will permit a “phase-in” of the revised EC on a voluntary basis. During the 12-month transition period beginning August 1, 2012, we will accept either the new form or the old form. Elevations certified after the last day of the transition period must be submitted on the new Elevation Certificate form with the expiration date of July 31, 2015.

At the risk of ratting on myself, I usually use Google Earth to get Lat and Long. at the center of the house. I check the approx. place where the GPS was set up out the open, and this is in reasonable agreement (seems to be within a few feet, but how close can we get with Goggle?) but I feel comfortable with a 66' error margin. But what might FEMA say?
