I am looking for a plate that can be mounted to the top of a survey pole and screw into the 5/8" thread. Something similar to this photo. Does anyone know where I can maybe purchase something like this? Thank you.

Your local machine shop can fabricate one to your specs.?ÿ It won't be Amazon pricing, though.
If you have to go to a machine shop, instead of having them do the whole thing from scratch it might be cheaper to get one of those speaker mounts as shown and have them make an aluminum insert cylinder, center drilled and tapped for 5/8-11.?ÿ Put it inside the speaker mount and drill 3 holes from the sides for screws to hold the cylinder in place. Only one easy part to make and no welding.
If I were wanting one of those, I'd probably try an aluminum plate and one of these adapters, re-tapped from 5/8-27 mic stand thread to 5/8-11 surveying thread.?ÿ A little klugy and maybe not enough thread left to hold, but worth a try.

SECO?ÿ makes all sorts of stuff like that for sure, but you might have to buy through a distributor.
These are leftovers from some we had fabricated a couple of decades ago, must have been for the external antenna used with a Trimble GeoExplorer.
The nut is 5/8-11, the washer is 2.5 inches diameter.?ÿ Holes in the washer are 9/64 inch on 2.125 inch circle.?ÿ All steel, if that makes a difference.

You might want to try a freelancer website like Fiverr.?ÿ My dad had some 3d printing (that's plastic of course, but you might find a machinist on that website) done for pretty cheap.?ÿ Just email them a sketch with your specs and they'll mail you the product.?ÿ For someone who owns the right equipment, it would be pretty easy I would think.
