Table A item 11(a) is a subset of item 11(b), and hence is included in 11(b). Erasing all of the underground lines shown in order to degrade the certification down to 11(a) is not a good idea.
The six survey prints I sent out were certainly signed and sealed.
The date of all revisions is shown, going back to the year 2005 when it was first surveyed by me.
There is a note identifying the title commitment, date, and name.
Providing a perimeter metes and bounds description on a platted lot or block is such a bad idea that it is not allowed in Arizona.
There is only one point of beginning, for the exception parcel, and it is shown on the survey drawing.
The widths of the adjoining streets are labeled.
There are no adjoining lot or parcel numbers to show since this site has public streets on all four sides.
I have used the correct 2011 ALTA certification, except you have omitted item 1 (monuments) which is required in Arizona, and 11(a) previously discussed.

All my ALTA survey contracts have two line items: 1.) a lump sum for the survey and 2.) an hourly NTE number for revisions and "settlement coordination".

Why NTE?
Think of it as a relationship building opportunity.
I still think it would have been more fun to have gone over and slapped the bingbong out of the paralegal.
Give them either time and materials at hourly rates, then handel all their needs and bill accordingly
OR
LUMP SUM + hourly for extras outside scope they request
NTE will be pushed for all it's worth by most clients you give it to. On some jobs it can eat all and more than your profit if you aren't a psycic tied into the future.
