FEMA MT-EZ Form

Has anyone downloaded and used the MT-EZ form that expires 09/30/2011?

Reason I ask is that when I open in MS Word, the fields on page 2 are incorrectly spaced so badly that there is not room for filling in the appropriate information.

When I try to download the other English language version, the Spanish language version pops up instead (as I don't read/write/speak Spanish, that version is not helpful to me).

I looked around, and did not find a replacement for the current version.

If you want, I have an unprotected version for you to use which eliminates the margin problems. Email me and I'll send it.

This morning, I called FEMA and asked about it. They said using the form that is dated as expiring in September 2010 would be fine, so I'll just use an old form and place the new data in it.
