Are there instructions somewhere for preparing an elevation certificate covering unimporved land?

We have to submit them as part of the permitting process for new construction, but I have never seen anything specific for unimproved land or new construction.
I always mention what the certificate was issued for and identify a site TBM in the comment section on the back of the page.
I have never heard a complaint.
Not sure what your application is, but you could check out the MT-EZ application as it can be filled out for removing a structure OR land defined by a metes and bounds description (Section A.3)
Vacant land, that the client will construct a new home on and they have inquired about an elevation certificate.
Section C1 of the standard Elevation Certificate allows you to indicate that the building elevations are based on Construction Drawings, Building Under Construction, or Finished Construction. If they already have a grading plan it shouldn't be a problem to pick off the necessary elevations.
Thats the same thing I do
You will need to prepare an EC for the proposed construction and one for post construction. Leave them a BM to set the floor elevation to meet the local code for building in a flood plain.
I think this is what is going on, we do that a lot here.

As Target said you will need them to remove land from flood plain. We use to do them to remove townhouse pads.

We've had quite a number of these. The lender is usually the source of the request. They want to know prior to doing the rest of the loan paperwork whether or not the site is or is not suitable or is easily modified to meet guidelines.
My experience on this is the same as Dan McCabe's.
