TBC, Note if it is standard, Rolled or anything other than standard. Preferably also get pictures so an offset to the TFC can be as close to realistic as possible. Even with that though I don't generally account for the 1/2" radius at the TFC

Tying top back of curb and gutter line at the face seems to be universal, as I suspected it would be. What is less universal is what is done to simulate (or not bother with) the top face of curb.
As you all may have guessed you where being used to settle a debate going on here in the office. So to avoid influencing the answers I have refrained from interjecting. I have followed the answers closely and thank all who commented.
