@sergeant-schultz Oh, I don't think I've ever seen a fillable one.  I thought everyone just typed it up in their .pdf viewer.

All of their fillable forms bite.  You can't fit the required data in the fields.  Don't get me started on the password locked Word documents they used to have.  When I get back to the office I try to remember to check the latest one I have to see if it is fillable.

To quote Nancy Reagan:  "Just say no" to elevation certificates.

@sergeant-schultz I'm not sure why this works, but here goes:  (This is using the Chrome Browser)

When you get to the Please Wait page, where it says "For more assistance with Adobe Reader visit http://www.adobe.com/go/acrreade r", click that link.  It may blow up and give an error, but you should see a blue bar appear across the top to the page that will say "This form contains fields that you can only fill in using the Acrobat desktop app." and there should be a button on the right end of the blue bar that says "Desktop", click the Desktop button. The Fillable form should then open automatically in Adobe Reader.  Also watch the top left of the form for a button hat says "Return to form" or "Form Instructions". This button toggles between the form and the instructions for completing the form which make printing the finished form easier.
