The answer to your question is apparently, "NO". I know I've never had such a request.

Yes, mostly for access easements. Table A Item 20 covers this.
Be careful when you do shopping centers. Sometimes the shopping centers are made up of multiple lots within a subdivision. If you are asked to do one of the lots in the shopping center, the access easement may cover ALL of the other lots in the shopping center. This would result in doubling, tripling, or quadrupling the scope of services.

Yes, for access easement verification and leased theater area inside a mall. Including seating capacity etc.

Yes. But not as a separate project, together with the ALTA of the fee parcel. Encumbrances on easements can be very important... they sure are to me on my properties!
