I haven't done a FEMA certificate since last November (FEMA form 086-0-33).
At that time, FEMA posted on their website that they were in the process of updating/reviewing the form and that surveyors should use the "old form" (2012 edition, expires July 31, 2015) until November 30, 2015.
I just now went to their website and of course it says nothing about any further update.
I hate explaining to clients about using an "expired" form.
Anyone heard about a due date for the new form?

I was wondering the same thing, when I just went to the FEMA site this was still there:
"The National Flood Insurance Program (NFIP) Elevation Certificate (EC) (FEMA form 086-0-33) is an administrative tool of the NFIP which is to be used to provide elevation information necessary to ensure compliance with to community floodplain management ordinances, to determine the proper insurance premium rate, or support a request for a Letter of Map Amendment (LOMA).
The EC form is currently under review at the Office of Management and Budget (OMB) and they have extended the expiration date of this form to November 30, 2015. Until their review process is complete, please continue to use the existing form. Upon OMB approval, FEMA will update this record with the new Elevation Certificate form."
I include the above statement in the "comments" portion of the EC's we do. (At least 25/month)
Apparently the new EC has been approved but not yet available for download.

Of course, that means the new EC will be downloadable after it too has expired. 😉
I would not fault anyone for using the old form still. They need to make it obvious when they finally change it.

I received a copy of the new EC, but when I open it, a message appears saying that my version (Acrobat Standard 9.5) is too old and some features might not work. Great, more money! What version do I need, 11?
I can enter text, but I still can't figure out how to put a picture in. Can anyone help?

They've created this form with Adobe Live Cycle and Acrobat Pro is incapable of editing anything in it. Filling in the blanks is the best you can do. Unfortunately the section for the photos was created as a text box which will not allow the insertion of an image.
Here's how you beat it into submission: Fill out the form then select print and choose [hl]Adobe PDF[/hl] as your printer. The resulting pdf will be flat (no fields) and you can go to [hl]Tools, Content Editing, Add Image[/hl], then insert images on the photo pages and text boxes for descriptions of the photos as needed. When finished, you can print to the [hl]Adobe PDF[/hl] printer again if you want to have a non-editable pdf to distribute or just send it to your regular printer for a hard copy.
Edit: The above works with Acrobat XI pro, similar workarounds will likely work with older versions.

I have Acrobat 9. I followed your example but did not find Content Editing. I printed to Adobe PDF, then Tools, Comment & Markup, Paste Clipboard Image as a Stamp Tool. This works in the Stamp block too. Still, we should not have to do this.
I shot a reply back to my County Flood Plain Coordinator noting the opening errors and the picture problems, and asked that she pass the information up the line. I also asked for a Word/Office version.
If enough of us do this, perhaps someone will listen and fix the problem - or at least make a reasonable work-around.
