Does anyone have an example of how they cross reference a separate cert sheet for an ALTA?

I'm thinking a note on the ALTA, "Additional Certification(s) on attached sheet."

Then on 8.5" x 11" letterhead, "The following Certification(s) pertain to sheets X through X of ALTA/ACSM Survey, File # (company file number.)"

Any thoughts?

I would recommend no note at all on the ALTA. The ALTA should be a stand-alone document with the standard ALTA certification meeting the ALTA requirements. The additional cert should make a one-way reference to the ALTA survey, but the ALTA survey shouldn't make a reference to the additional certification.
