Should section b item 8 for the flood zoneor zones reflect the horizontal location of the structure relative to the SFHA shown on the FIRM? ....or reflect the zone from actual surveyed elevation?

Used the location scaled from map, the map is used to determine Zone, if the ground is higher than the indicated BFE you can apply for a LOMA using EC as a substitute for Elevation Form MT-1 Form 2
