Elevation Certificate - Section A

When hired to complete an elevation certificate, how many of you routinely complete Section A (or any other sections) beyond Section C which requires your certification?

The same with me - A, B, C, and D every time. If you don't fill in Section A yourself then how would you guarantee that the certified elevation information in Section C isn't applied to the wrong structure or site, either by mistake or on purpose?

Yep, A, B, C & D every time.

The certification in section D states "I certify that the information on this certificate represents my best efforts to interpret the data available."

My interpretation is that the certification applies to the entire certificate, not just section C. I'm not going to let someone else fill in A and B when I'm the one signing in D.

For items B1 thru B9 I get a letter from the FPA. I know of three cases where a surveyor used an expired firm. One was out of date by several weeks the other two by several months.

Because of LOMR(A) ABFE and community free board requirements I make sure the FPA gives me the required elevation when I set a bench mark for new construction.
