I am running AutoCAD Civil 3D 2015 with Carlson Survey 2015 add-on package.
In my "Import Text/Ascii File" dialog box, I have the coordinate order set for point, northing, easting, elevation,description and several note fields.
A typical point in my CSV file looks like:
40025,97988.013,2016618.196,1331.307,mon,undisturbed,2-Jan,found,iron rod
In the past, when I would import a CSV point file and use the draw-locate function, it would place a point block with the point number, description, elevation and then it would add the notes on individual lines of text on a note layer underneath the point block. For some reason it doesn't seem to be labelling the notes anymore. Is there a variable somewhere that got turned off that I'm missing? I know it used to work.

Based on what you've posted:
P=40025
Y=97988.013
X=2016618.196
Z=1331.307
D=mon
N=undisturbed
N=2-Jan
N=found
N=iron rod
In the title of your thread, you have "...D,N,N,N,N,N" (5 N's as opposed to 4) which might be causing the problem. I hope this information helps.
Actually, my format is?ÿP,Y,X,Z,D,N,N,N,N,N,N,N,N,N,N
We collect a lot of metadata which goes into a very robust GIS for our client. I don't think the presence or absence of data in the additional fields has ever been a problem.
not sure if you already fixed it but I believe your solution would be to create a new label style and add each of the fields to the label. Have you tried that?
Do you have "notes" checked in the Draw Locate Points Dialog??ÿ
