Convert PDF or Image File to CAD type File

I was wondering if any of you guys had any experience with converting a PDF or image file (TIF, JPEG, BMP, PNG, etc.) to a CAD line file (DWG, DXF, DGN, etc.)? I have found a few programs that seem to do it with varying levels of success, but I was looking for one that would put items on different layers/levels. I guess this would need to be done by color, since there isn't much else to distinguish between layers/levels once in a PDF or image format file. Do you guys have any programs you have used with success to do this? The ultimate input file type and output file type aren't that important (I can convert to just about any image format, and can read most CAD file formats with AutoCAD or Microstation). I know there are a lot of programs that seem to call this raster to vector conversion also.

First hand experience, comments and suggestions welcome! Thanks.

I have tried many times to do this but I always end up just tracing it.
