My state does not require a written survey report, but I am in a situation where I think a written report would be super useful. Would anyone have a sample report that they did not mind sharing for an example or guide?

NC has a decent example (attached) for a report relating to boundary surveying.
Who, what, where, when, why, and how will get you most of the way there. It largely depends on the purpose of your survey, but I'd always include metadata that shows the reference datum and when the fieldwork was performed.
When I draft a report for lidar surveys, I focus on details pertinent to ASPRS standards (sensor, Height AGL, RMSE accuracy etc.). When I draft a report for a monitoring well survey, I make sure to include a chart of the top of casing N, E, and Zs. Just imagine someone looking at the report ten years from now, and include all the info they'd likely want to know to retrace your steps or evaluate your work.
