Don't have one. 99% of my contracts are on letterhead. It includes the essentials: the parties involved (client/surveyor); reference to the project; scope of work; what I will provide (included); what is not included (but obviously can be for additional fee); terms of payment; my signature; their signature, including printed name and title; date. Usually fits on one page.
Sometimes they come back with their contract that always contains the legalese I think you're looking for. No problem, but I always condition on my initial submitted one for scope and included/not included items, at a minimum. Otherwise, just negotiate again.

The book "A Pocket Guide to Business for Engineers and Land Surveyors" has a short form contract in it. The book is worth the price without it.
