In the past, I have been responsible for layout and inspection of pile driving operations.
It can be a stressful job. I try to stay as far away as I possibly can, while still being able to monitor the depth marks. However when things fall off a crane, they can bounce outwards further than you think. The driving operation really beats the heck out of the equipment and I suspect that in this situation, something failed.
We use Micarta/Conbest cushion on the hammer with nothing in the cap and have had very good results. Not so much with plywood.
