Does anyone know if there is a revision to the MT-EZ application? The current form expires Sept 30, 2010 and FEMA's website does not have a revised form (at least that I can find).

This isn't the first FEMA form to be used after it's expiration date.
To the best of my knowledge the form is still valid.
