I follow one practice consistently. Can you tell what it is from this bit of a transcript of a deposition I gave recently:
>Q. Mr. McMillan, I'm going to hand you what I've marked as Exhibit 6 to your deposition and represent to you that this is an affidavit that was attached to a motion for summary judgment. And is that, in fact, your signature on Exhibit 6?
>A. Yes, that is.
>Q. And did you read this affidavit before signing it?
>A. Actually, I wrote that affidavit.
I don't care who is asking for an affidavit. I always draft them in my own words so that I can craft the language using terms familiar to me to say what I intend to say.

We call them Declarations and I do the same, write it myself.
Must say as well I do not sign blanket Powers of Attorney either unless I write it and specify the limited powers intented thereby.
