Here is some of what the OCS sent me a few weeks ago (never attached files on this site). Two files are too large to upload (BPELSG 2016 LT2000.dwg & BPELSG 2016 R14.dwg).

You may also want to go to the source: http://www.bpelsg.ca.gov/pubs/forms/corner_record.pdf as there have been some minor reformatting to the form by BPELSG.
I think on the next one I'll fill out the form in the PDF and print the drawing to PDF then combine them in Acrobat. I have an AutoCAD drawing I've been using for years but it's now obsolete.

Seems the Board supplied form comes up short on the aspect of a full electronic submittal to an agency.
The other issue is that they kept the "City of ___________" verbiage. I wish they had incorporated a pull-down to choose from applicable situations; such as Town of, Unincorporated Portion of...
Where were all of these comments when the new form was being revised by the TAC and the Board? It was a public process.

If you have a suitable electronic version of your seal and signature that complies with Board Rule 411(e), it's simple to insert those, for example in AutoCad or Adobe Acrobat
