McDonald's aint health food for sure, but there had to have been some loose "science" behind the Spurlock film. It is well addressed in Fathead.
I think both guys took different approaches. It sounds like the Iowa teacher tried to only eat the healthiest things McDonald's offered, (Which McD gave him for free, BTW) and strictly limited his calories, while increasing his exercising.
The "Supersize Me" guy took the approach of eating anything McDonalds offered him, and basically saying "Yes" to anything the McDonald's employee asked him.

When it comes to fast food, I normally find that trying to eat healthier costs a lot more. At McDonald's, you get like 4 or 5 "McDoubles" for the cost of a Salad or Grilled Chicken Sandwich. All the stuff on their Dollar Menu is empty calorie crap, but that is what they push the hardest.
They say it costs about $1.50 more per meal to eat healthier at fast food places, but they also say that more than pays for itself in healthcare costs.
I like Chick-Fil-A and Taco Bell for fast food lunches. It's pretty easy to get a better meal at those places. Taco bell might be the only place where the cheaper menu items (fresco style tacos and bean burritos) are the healthier chooses.
Couldn't agree with you more! I get the fresco bean burritos all the time partly because they are healthier but mostly because I like them and you can't beat the price.
Fathead is a separate documentary that points out some blatant monkeying with the data in or out in Super Size Me.

Never could understand why people bad-mouth McDonald's food compared with more expensive restaurants. Do they really think the food is healthier if you eat at a mid-priced restaurant like Applebees or even expensive restaurants?
What I like about McDonalds, is you can buy exactly as much food as you need. Other more expensive places seem to give you huge portions which is far too much food.
