I meant to reply to you earlier, but I've been trying to locate my master documents to see if I did leave out "Not". It was 1997 or 1998 when this work order document was put together and since I'm now retired, never really use it anymore.

This does not go on survey plats. This is an excerpt from my Work Order document. It is a compilation of another firms work order, writings of Jerry Broadus, Ted Madsen, and others. The original document was prepared in the mid 1980's and then modified in the mid 1990's. Since I am now retired, I no longer need to worry about using it. I posted the excerpt because it was similar to Scott Freshwaters post.
