New FEMA Elevation Certificate

Just a quick review of those that made alternatives, thanks. Unfortunately, they lack the links for the states, date format and the picture and seal links. Basically, when you pick on the seal square, you get an option to go find a jpg file of your seal to insert, the same with the pictures. I manage to keep this functionality with the last certificate and just extracted the pages needed for the form. The other issue is with the comment section, it doesn't word wrap. 

I just used the FEMA form and it worked fine. I can just printed a PDF of pages 3-9 for the final certificate, print the signature page, scan it back into the finished form. Its no that much different then what I had.

Also, the N/A does not show up in the elevation spots anymore. You can't even put them in manually.

You should be able to make those changes yourself using Adobe Acrobat with the "Prepare Form" option

Update: Thanks. I sorted it out. Didn't realize he had Adobe Pro, so used that to add a Javascript to add the jpg files. Thanks again for the form!!!

I am assisting a surveyor client. Thanks for this form! It works fine on my computer, but having trouble on his with just the photo portion. Does the action in the button use java to add a photo? I have Java, he doesn't. Thanks in advance.

In a FEMA webinar today and someone from FEMA in the chat said they will release a version without instructions, after they fix the problems with the current form. like "Per the instructions, non-applicable elevations in section C.2 are to be filled in with an " N/A" designation; however, on this new form, these entries will only permit numerals and some punctuation."

Please hold your breath.
